The trainee couldn’t have identified this. And the opposite pilot couldn’t have identified the trainee was to do supervised flying beneath a coaching captain. This was an oversight by crew administration system (CMS) which couldn’t make sure the authorized complement of crew. The 2 pilots realised what had occurred and filed a voluntary report with the bottom supervisor,” stated sources. The airline then voluntarily reported it to the regulator.
A senior DGCA official stated the 2 pilots have been off-rostered (taken off flying obligation). The regulator is learnt to be analyzing whose oversight led to this mix-up. Some AI officers have been referred to as to the regulator’s workplace to repair accountability, say sources.
